Athletic Pre-participation Requirements for all Freshmen and Transfer Athletes

On behalf of the Athletic Training/Sports Medicine Department, welcome to Moravian College. Another school year is soon to begin and along with the beginning of the school year is the beginning of the athletic seasons.

The Athletic Training/Sports Medicine Department is responsible for the prevention and care of injuries occurring in the Intercollegiate Athletic Programs. The services of this department are available to all students participating in the College’s Athletic programs.

To help in the prevention of injuries all first-year athletes are required to have a pre-participation physical. . As an incoming student, Moravian College requires you to have an entrance physical. In your admissions packet you received the required physical form. This form needs to be completed by your physician. The original form is to be submitted to the health center and a copy of that form is submitted to the Bob Ward in the Athletic Training Department.

Please complete the following:

1. Complete and return the following forms:

a. Release of Medical Information and Permission to Treat form (PDF)

b. Athletic Participation Waiver form (PDF)

2. Review the Student Athlete Accident Insurance Policy (PDF)

Please read the Student Athlete Accident Insurance Policy. It explains how the athletic insurance policy works. We also suggest that you print a copy of this document for your records. Please also make sure that when you come to college you bring your health insurance card with you. If you have any questions about this policy please contact Bob Ward at 610-861-1537.

3. Complete on-line HomeLink program. HomeLink is an on-line database which you will use to submit general information, (home address, emergency contact information, insurance information, etc.). A user ID and password are needed to access this program. You will receive a letter, in the mail, which will contain your user id and password.

1. Down load complete and return the following forms:

a. Release of Medical Information and Permission to Treat form (PDF)

b. Athletic Participation Waiver form (PDF)

c. Returning Athlete Medical History Survey form (PDF)

d. Updated Insurance Information form (PDF). You only need to complete this form if your health insurance information has changed since last year.

f. Returning athletes do not need to complete the HomeLink program

All forms must be submitted and the HomeLink program completed by July 15th.
