Vaccination Policy An immunization record is required of all students. It must be completed and signed by the health care provider. We also require a tuberculosis screening questionnaire. All students are required to answer a few simple questions regarding their risk for tuberculosis. The questionnaire must be reviewed and signed by the student’s health care provider. If needed, a TB screen must be done. We highly recommend vaccination against meningococcal disease; a student who refuses must sign a declination statement and return the form to the Health Center. We recommend Gardasil, hepatitis A and flu vaccines.
