Nursing Portal Requirements for Accelerated Bachelors of Science First Year

(Post Baccalaureate Cohort Year 1)

The following Forms must be submitted via Document Upload in Student Health Portal  by June 1st

PLEASE NOTE: The Nursing Department is NOW using the student health portal for all documentation and uploads and NO LONGER using AMOS. Use this link PORTAL LINK to get to the student health portal 
 

Web Portal Document Uploads

  1. FBI Cogent Fingerprinting: Follow the instructions on website https://uenroll.identogo.com. You will need to first put in service code 1KG6ZJ to get started. The second code is the Facility code 1665. Please make an appointment to schedule for finger prints. The cost will be paid by the student not Moravian University. Please then upload completed Fingerprint form to student health portal utilizing the document upload feature using the Nursing FBI FINGERPRINTS document type.
  2. Pennsylvania Access to Criminal History (PATCH): Instructions and registration for this clearance can be found at: https://epatch.state.pa.us/Home.jsp . Once all the required information is complete please upload the form to student health portal utilizing document upload feature using the NURSING (PATCH) Criminal History Form document type.
  3. Child Abuse Clearance: Go to www.compass.state.pa.us/cwis/   Create an Individual account and follow directions from there. Complete the form, “Child Abuse History Clearance.” Be sure to follow the instructions listed on the top of the form. Once formed is complete please upload form student health portal utilizing the document upload feature using the NURSING CHILD ABUSE CLEARANCE document type.
  4. Download the PDF from this link, ‘New Nursing student health packet’ Take it to your medical provider then upload to the student health portal (Moravian.studenthealthportal.com) and utilize the document upload feature with the label of ‘Physical’ in the document type.
  5. Annual INFLUENZA vaccinations are mandated by the clinical sites all students must be vaccinated by October 15th.  Once you have received the vaccine upload documentation to the student health portal utilizing the document upload feature using the ANNUAL INFLUENZA document type.  If unable to be vaccinated please contact the CLINICAL COORDINATOR for the Nursing Department by SEPTEMBER 1ST.
  6. Health Insurance and CPR Certification Cards - Upload a copy of both sides of your insurance card utilizing the student health portal document upload feature using the Insurance Card Front/Back document type. Also upload a copy of your CPR certification card utilizing the document upload feature of the student health portal and the NURSING CPR CERTIFICATION document type.

Web Portal Forms must be completed by June 1st utilizing the student health portal - click on MY FORMS

  1. Medical History form- you complete regarding YOUR personal medical history and some family history
  2. Nursing Immunization History form- more detailed immunization record gets completed online PLUS uploaded in document upload feature
  3. Current CPR certification form (note that the date must be valid through the entire academic year)
  4. TB Screening form – answer questions about TB symptoms
  5. Clinical Experience Sign-Off documents – you read and initial/date at each option to acknowledge your understanding as stated on the form.
