• Bachelors/Masters of OT
• 3.0 GPA
• Resume/CV
• 2 LOR from professional references; preferably 1 from an OT
• Short essay/personal statement/letter of intent
• Moravian applicants: licensure and certification
• External students: license and certification and 1 year of practice  
• Transcripts
• Interview (virtual)
