Financial Aid Verification Forms
Some students will be selected to provide additional information to finalize their financial aid eligibility. The Office of Financial Aid Services may require additional forms to be completed or request copies of tax or other income documents to verify the information reported on your FAFSA. You will be notified via US mail or via your Moravian E-mail of any additional forms you may need to submit.
For security of your personal information, please do NOT email confidential data or any documents containing personally identifiable information. Documents and private information should be submitted electronically via XMedius SendSecure, our secure file upload portal. Documents may also be safely transmitted via our dedicated fax (610-861-1346) or mailed to Moravian University's Office of Financial Aid Services (1200 Main Street Bethlehem, PA 18018).
Please ONLY submit these forms if they were requested by the Office of Financial Aid Services.

Authorization to Release Financial Aid Information
The Family Educational Rights & Privacy Act (FERPA) limits the kinds of information that the Office of Financial Aid Services can discuss with parents and third parties of students. Students: Please submit a FERPA Permissions Form on AMOS (the form is on the College Students Tab under Forms) to authorize individuals other than yourself and your FAFSA parent(s) to speak with Financial Aid Services on your behalf.
